Our Social Services Program strives to provide quality and culturally appropriate Social work services to American Indians and Alaska Natives. The overall purpose is to help Indian clients maintain, regain, or enhance their level of social functioning when their ability to function is affected by actual or potential illness, disability, or injury. The content of these services range from prevention and education to helping clients deal with major changes in social roles, future expectations, and life goals precipitated by changes in health status.

Forms:

What do I do with the forms?

  • We do not accept electronic signatures.
  • Documents must be printed and signed with a “wet” signature (either by printing and filling them out by hand or filling them out digitally before printing).
  • Completed packets can be delivered in person or by mail.
  • For first-time registrations, the original birth record must be brought in person for copying.
  • For all other documents, copies or emailed scans are acceptable.
